Client Consultation Policy
By signing this form, you agree to the following terms and conditions that govern your care and our working relationship.
Scope of practice The nutrition advice, gut microbiome interpretations, and meal plans provided are for general health and wellness purposes only. They do not constitute medical advice and are not intended to diagnose, treat, cure, or prevent any medical condition. You are encouraged to continue working with your GP or relevant medical specialists alongside your nutritional care.
Appointments Consultations are available both in-person and via telehealth. It is your responsibility to ensure you are on time for your appointment. Late arrivals may result in a shortened session, with the full consultation fee still applicable.
Cancellation & rescheduling We require a minimum of 24 hours notice for any cancellation or rescheduling of appointments. Cancellations made with less than 24 hours notice will incur a fee of 50% of the scheduled appointment cost. Failure to attend an appointment without notice will be charged at 50% of the full appointment fee. We understand that emergencies happen and will consider extenuating circumstances on a case-by-case basis.
Meal plans & written reports Meal plans and written gut microbiome summaries are prepared specifically for you based on the information you provide.
